If erections start firm but soften within minutes, or tablets work less every year, the problem may not be blood flow in but blood flowing out. A venous leak lets blood escape the penis too quickly. Assoc. Prof. Dr. Tuncay Taş diagnoses it with Doppler ultrasound in Istanbul and treats it at a fraction of Western private fees.

During a normal erection, the erectile chambers fill with blood and press the small draining veins flat against their firm outer wall, the tunica. This seal, called veno-occlusion, traps the blood and keeps the penis rigid.
In a venous leak, also called veno-occlusive dysfunction, the seal fails. Blood keeps draining out as fast as it flows in, so erections are weak, short-lived or lost during sex. The commonest causes are:
Typical signs are an erection that fades within minutes, a better erection lying down than standing, and tablets that help less over time.
What published studies and guidelines tell us about diagnosis and outcomes:
Doppler blood flow between heartbeats that, with a strong arterial peak, points to a venous leak.
EAU Guidelines
Long-term success of vein ligation surgery, as new drainage veins tend to open.
Diehm et al., 2023
Clinical success of venous embolization across 16 published series of 212 men.
Doppalapudi et al., 2019
Patient satisfaction with a penile implant, which bypasses the leak completely.
AUA Guideline
In an early Ankara series, erections weakened again within 15 months in 21 of 25 men after ligation. That is why careful diagnosis and honest expectations matter more than the procedure itself.
Weak erections have many causes, so the leak must be proven before any treatment is chosen.
Treatment moves from the least invasive step to the most reliable, depending on how severe the leak is.

Your symptoms fit a venous leak, but only a scan can confirm it.
Based on EAU and AUA guidance on veno-occlusive dysfunction. Educational only; your treatment is decided after examination and Doppler testing.
How a typical stay in Istanbul runs:
In many countries the scan, the vein procedure and the implant sit with different departments. In Istanbul one specialist manages all of it.
Assoc. Prof. Dr. Tuncay Taş will tell you honestly when a vein procedure is worth trying and when an implant is the better use of your trip. If an implant is the answer, see which penile implant is best.

Closing the leaking veins helps many men, but the benefit often fades as new drainage veins open, and long-term success after surgery is only around 25%. A penile implant is the only treatment that bypasses the leak for good.
With a penile Doppler ultrasound after an erection-producing injection. Strong arterial inflow combined with an end-diastolic velocity above 5 cm/s points to a leak. CT or dynamic cavernosography then shows which veins leak if a vein procedure is planned.
At Dr. Tas Clinic in Istanbul, a Doppler diagnosis costs $300 to $500, venous embolization or ligation $3,000 to $5,000, and a penile implant $5,000 to $12,000. That is usually 50 to 70% less than private fees in the UK, US or Western Europe.
Two to three days for diagnosis alone, four to five days for embolization or ligation, and five to seven days for a penile implant.
Yes. In younger men it is often linked to a congenital vein pattern, an old penile injury or Peyronie's disease. Young men with a single, well-defined leak are the best candidates for embolization or ligation.
Low-intensity shockwave therapy may improve mild cases by supporting the erectile tissue, but it does not close large leaking veins. It works best alongside tablets in men whose erections are only partly affected.
